This form is only to be completed once you have a confirmed place at the after-school club for the Autumn term.

Activities are chosen on a first come, first-served basis. Please note you will be committing your child to these activities for the whole term. Children cannot move in and out of activities as we must staff accordingly.

Please note if your chosen activity is full your child will automatically move into freeplay.

Please select the preferred activity for each session your child is attending.
