This form is only to be completed once you have booked a place at the after-school club for the Autumn term via SchoolCloud.Activities are chosen on a first-come, first-served basis. Please note you will be committing your child to these activities for the whole term. Children cannot move in and out of activities as we must staff accordingly. Please note activities may be subject to change if there is low demand for a particular activity. If your chosen activity is full, your child will automatically move into free-play.
